2011/2012 Registration Form

2011/2012 Instructions

Thank you for your attention to this form! Completing the registration form accurately and in a timely manner is very important as we have over 1,000 students to place.

Attention Public School students!

Students in Grades K-10 need to be registered every year
Please register promptly. All classroom lists and future mailings are generated from completed registration forms.

Attention Catholic School students!

Catholic School students need to be registered in Grades 2, 9 & 10
Catholic School students in Grade 2 need to register for Eucharist and Reconciliation preparation. You will receive a schedule in a future mailing.

Please call if you are experiencing a financial problem. No student will be turned away due to an inability to pay the registration fee.

Please consider becoming a catechist! Our program depends on volunteers. You will receive training and support. It will prove to be a rewarding experience for you! All teachers receive a 50% discount off the total registration fee. Teachers are not required for the “Households of Faith” program.

PLEASE CHECK OFF ALL APPLICABLE SECTIONS ON THE REGISTRATION FORM:

KINDERGARTEN: meets 1st and 3rd Saturdays of the month 11:00 AM to 12:00 PM

GRADE 1: meets every Saturday from 9 AM. – 10:15 AM or 11:00 AM to 12:15 PM

PLEASE INDICATE YOUR PREFERRED START TIME.

GRADE 2: meets every Saturday from 9 AM-10:15 AM or 11:00 AM-12:15 PM

Grade 2 students must participate in the General Program, Eucharist, and Reconciliation programs. (Not eligible for “Households of Faith” program). PLEASE INDICATE YOUR PREFERRED START TIME.

GRADE 3: meets every Sunday from 10:15 AM-11:30 AM or every Wednesday from 4:15 PM – 5:30 PM. PLEASE INDICATE YOUR PREFERRED DAY AND TIME

GRADE 4: meets every Sunday from 10:15 AM – 11:30 AM or every Wednesday from 4:15 PM – 5:30 PM. PLEASE INDICATE YOUR PREFERRED DAY AND TIME.

GRADES 5, 6, 7 & 8: GENERAL PROGRAM ONLY – Meet for 1½ hours twice a month (This does not apply to children in the “Households of Faith” program)

PLEASE NOTE: IN ADDITION TO REGISTERING ON THIS FORM, parents of Grade 5-8 students must come to our in-home class sign-up weekend. Catechists for these grades choose the time and day of the week that they wish to teach. Parents need to choose a class day and time that best suits their child’s schedule.

THESE CLASS SIGN-UPS ARE SCHEDULED IN THE CHURCH GATHERING SPACE ON:

Saturday, August 27, 2011 from 10 AM – 12 PM
Sunday, August 28, 2011 from 10 AM – 12:15 PM
Monday, August 29, 2011 from 6:30 PM – 8 PM

If you are unable to attend one of these dates, we suggest you send a friend as classes fill quickly!

CONFIRMATION – A TWO YEAR PROGRAM

All students in Grades 9 & 10 (public, private and Catholic schools) need to register for both years (Year 1 and Year 2) of the Confirmation program. Catholic school students in Grade 9, however, are not required to attend classes at St. Mary’s or pay the registration fee. We need your registration information for mailings, retreat information etc.
Catholic school students in Grade 10 are required to pay the registration fee and attend classes from January – March.

GRADES 9: Classes meet on Sunday evenings from 6:45 PM – 8:15 PM twice a month

GRADE 10: Classes meet on Sunday evenings from 6:45 PM – 8:15 PM twice a month

Please do not register students who have completed Grades 9 & 10 of the program and are scheduled to receive the Sacrament of Confirmation in the fall of 2011.

HOUSEHOLDS OF FAITH PROGRAM (Family Program)

This is available to all families with children in public school, in Kindergarten, Grade 1, and Grades 3-8.
Grade 2 students are not eligible for the Households of Faith program.

Families in this program meet once a month on the 2nd Sunday of each month from 12:30 PM – 2:30 PM

RCIC (Rite of Christian Initiation for Children):

All students beyond Grade 2 who have not received the Sacraments of Eucharist and/or Reconciliation must attend these preparatory classes.
Class times TBA.
Students in this program are exempt from regular religious education classes for the year.

Please check off “RCIC” and the sacrament(s) your child needs on the registration form.

NOTE:

Please indicate on the back of the form any medical or educational issues that might be of concern to you regarding your child.

Fees

Registration fees for weekly general program, Households of Faith program, in-home program and high school program are as follows:

first child	$70.00
second child	$60.00
all other children	$50.00
max for each family	$180.00

Sacramental Programs:

Catholic School Eucharist Program	$25.00
Catholic School Confirmation Program	$70.00

Fees are decreased by half for volunteer Catechists
